After getting your joining letter signed by the HOD of Surgical unit-1 and opening a bank account:             

Know your schedule - The OPD,OT days,Free-days,emergency and cold calls,their timmings. Introduce yourself to all the members of the department Get to know your duties. How you are to carry them out.

 

Your Duties - OPD Note that the patient is seated on a stool placed right next to your chair. Note that the patient brings a slip issued from the counter that has his/her name,age,medical registration number and surgical unit-1 written on it. On the space provided below,write his/her presenting complaint towards one side. Diagnose the disease,condition and write down the investigations or treatment. Make sure you write it down in the register as an entry.This includes the medical registeration number,the name of the patient,diagnosis and if to be opereted then what day.For e.g 'On wednesday's list.' Decision of the operation to be made is based on local or general aneshthesia.               Acknowledge the patient of the date of admission,the expenses etc. 

 

 

Your Duties - WARDS Admit the patient.Write down the plan.Take history. Put down the morning,evening and night notes everyday on your allotted beds.And for all the beds when on call. Write down the consent,labs,aneshthesia fitness.Keep the patient NPO sometime prior to operation. Call to medicine,ENT or any other department for evaluation prior to surgery,if needed. Post-operative management includes:Checking the drains everyday.Also,Foley's catheter and NG readings.Note the antibiotics,PPI cover,pain killers and fluids given.Keep NPO for sometime.Then depending upon the operation e.g Appencectomy allow sips of water after one day,juice,semi-solids and then soilds. Write the discharge letter when required.The management on discharge is to be written by the PGR. Your Duties - EMERGENCY For every RTA (road traffic accident) case,see for any laceration.If there is any, then suture. Write down all that is needed for suturing e.g prolene 4/0 for a limb laceration.Also,gauzes,powdered gloves,a surgical blade (to shave the head hair),xylocaine,disposable syringes,a sticking tape etc. For a head laceration,shave the hair with a blade.Inject the local anesthesia on either sides of the wound.Then suture. Using a gauze stop the bleeding.Then with help of a sticking tape do the dressimg. Write down the medicines on the slip provided prefably a pain killer like Dicloron in combination with an antibiotic like Augmentin/Brufen etc. Handle other cases like intestinal obstruction with vigorous vomiting etc by thorough examination.Check by auscultating for exaggerated bowl sounds.Know the contents of vomit and level of obstruction.Also,for any blood in the patient's stool.Write the investigations and medicines accordingly.

 

Your Duties - COLD CALLS All the ward work is to be done on the this very day.

 

Your Duties - OPERATION THEATRE DAY Wear your OT slippers before entering the theatre.Wear your scrub. Prepare patient for operation.Get the perforators marked in case of a varicose vein patient.Call the anesthetist.WASH-UP for assisting an operation. Assist the operation.You can now touch the instruments. Take your gown and gloves off. Register entry. You can now wear your daily wear after you're done assisting surgeries. Imprint
